Once we receive approval to provide home health services from your doctor, our professional and caring staff will visit you at your home to talk to you about your needs and ask questions about your health. We will communicate our findings to your doctor. Our team will work with you and your doctor to develop your plan of care. Your personalized plan of care lists what kind of services and care you will receive for your health problem. You and your family will be involved in any decision about your plan of care and treatment. Your plan of care includes the following: • What services you need • What health professionals should give these services • How often you will need the services • The medical equipment you need • What results your doctor expects from your treatment Each patient under our care will have a dedicated case manager to supervise his or her care and advocate for you.
